To provide my services as a volunteer for LHVC without any expressed or implied promise of salary, compensation or payment of any kind.

To comply with the ethical and humane treatment of all animals in my care while serving as a volunteer for LHVC.*

My position as a volunteer will not result in any employment based benefits--which include but are not limited to: workman’s compensation, insurance, stock options, vacation or sick time.*

Read and understand, to the best of my knowledge, the policies and procedures of LHVC and will comply with the standards set forth by the clinic.*

LHVC treats potentially abused, injured, or sick animals of which the temperaments and overall demeanor is unknown. I will not hold LHVC accountable for any injuries I receive while handling animals during the course of my volunteer duties.*

I understand and agree to assume risk while performing any and all activities associated with LHVC.*

To not hold LHVC accountable for any damage to my property while serving as a volunteer.*

Failure to comply with any or all of the obligations as outlined in the Volunteer Agreement, or for any reason whatsoever, while performing my duties as a Volunteer with LHVC, it is LHVC’s sole discretion to immediately terminate my services.*

Dress Code*

Allowed: Closed toed shoes, pants/jeans, volunteer T-shirt or loose fitting top. Scrubs preferred but not required.

Not Allowed: Torn jeans, open toed shoes, sandals, thin tank tops, mid-drifts, skirts, or shorts. Please understand that this is for your safety when working around animals.

I agree to the dress code and understand that I will not be allowed to volunteer if not dressed appropriately.

I acknowledge that I have read and fully understand the terms and conditions of the foregoing Volunteer Agreement and will comply with the policies as described above.

I hereby agree to accept a position as a volunteer for Legacy Humane Veterinary Clinic (LHVC). As described by the State of Texas, my position as a Volunteer is performed willingly and without pay. I release all liability to LHVC for any actions performed hence forth, and therefore willingly and knowingly take responsibility for my actions as a Volunteer for LHVC.*
