Minimum payment required: $50 deposit
Thank you for submitting your request for Heartworm Treatment at LHVC. Staff will be in touch to confirm your appointment needs and schedule a time/date accordingly. The $50 will be deducted from your grand total at the time of your appointment. The balance due will be paid by Adopt My Heart. If we are unable to accommodate your pet’s needs, the $50 deposit will be refunded to you. Please review the information below for pre-appointment paperwork and additional services that you can add to your upcoming appointment. These services can be added at the time LHVC contacts you to confirm your appointment or during your upcoming appointment. Please note that any added services outside of the heartworm treatment will not be covered by Adopt My Heart and will be due at the time of the appointment.

*Please be aware that the base price for your pet’s Heartworm Treatment, as outlined on this form, may not encompass all potential charges associated with the procedure. Additional charges may be incurred at the time of the treatment.
Before Treatment
There are no food/water or exercise restrictions prior to the Heartworm Treatment.
Required Information:
Proof of current heartworm test with results. If records are not available, a heartworm test will have to be completed on the day of the appointment for an additional $20.
Proof of current rabies vaccination is required If your pet has already been vaccinated against rabies, bring the original, signed rabies certificate to the clinic with you. A rabies tag is not acceptable proof of vaccination. If you do not have proof of rabies vaccination, we will vaccinate your pet at an additional cost of a regularly priced Rabies vaccine.
Other Medical Information:
You are welcome to bring any other medical records with you to help us determine your pet’s other vaccination needs, but it’s not a requirement. Please see the vaccine and service costs on our Vaccination Services page.
